Job description

Calderdale Council is the most improved adult social services Directorate in Yorkshire and Humber. Our work has been nationally recognised winning a series of awards in recent years.  We currently have an opportunity for a Social Worker to join the Hospital and Intermediate Care Social Work Team.

 

The team support individuals at times of crisis and change. You will join an experienced and supportive team, undertaking assessments to help facilitate good discharges from Calderdale Royal Hospital, Huddersfield Royal Infirmary and intermediate care settings. The role involves close liaison with health colleagues, other professionals and organisations to promote health and wellbeing.

 

The team are strong advocates of the Mental Capacity Act and Human Rights,

ensuring that the human rights of individuals are at the forefront of service delivery. Individuals are supported to remain in control of their lives and make choices that enable their independence wherever possible.

 

The team works in genuine partnership with people, their families and carers.

 

Social Workers receive an extensive Continuous Professional Development offer.
