A refreshed sense of focus and energy is taking our Adult Services in an exciting direction, and we want you to be part of these changes we know are needed. We are now seeking an enthusiastic Social Worker, passionate about adult social care and providing a high-quality service and is keen to develop their professional experience and knowledge. You will join our Continuing Health Care Team which works collaboratively with our Clinical Commissioning Group. This team is part a comprehensive multi-disciplinary team.
Duties will focus largely on undertaking direct work on continuing health care and supporting colleagues. You will ensure that social work interventions are person centred, uphold rights and that all decisions are necessary and proportionate. This is an opportunity to specialise in a specific area, to be a member of a small, supportive team and to undertake some independent working.

You must have a sound working knowledge of The Care Act 2014, Adult Safeguarding policy and procedure, the Mental Capacity Act 2005 and the National Framework for Continuing Healthcare and NHS funded Nursing Care. It is also essential that you have thorough understanding of empowerment and risk enablement.
