Job description

We are recruiting a Mental Health Social Worker to join the Integrated Community Mental Health Team based at Gibraltar House in Northallerton.

The post involves having responsibility for assessing, planning, implementing and evaluating social care for individuals in a community setting. You will support people within the secondary care pathway of the team, using a recovery model approach to help maximise people's potential to maintain their dignity and independence in their own homes and communities.

In the case of residential rehabilitation placements, you will assess the appropriateness of the placement, and care manage the transitional arrangements around admission and discharge. You will monitor, review and reassess individual care plans while acting as a Mental Health Social Worker.

You will provide professional support in terms of interventions with individuals, families and groups and jointly work and communicate with staff across the primary and secondary care interface. You will also liaise with non-statutory organisations to meet service user /carer needs. You will work in partnership with members of the multi-disciplinary team e.g. medical staff, occupational therapists, psychologist, social care staff, nursing care staff and students on placement - delivering the requirements of the Care Act and other statutory duties such as Adult Safeguarding.
